Select the country or region in which your facility is located. **Sipfacilitytype** **Facility Type** *(Ref ID - sipfacilitytype)* 1. Select your facility type(s) from these options. Based on the selection(s), you will then see additional questions for product categories, materials used, facility processes, and industry sector. #### **Facility Type Option** #### **Examples** **Finished Product Assembler** Finished goods production/ final product assembly (e.g., shirts, pants, footwear) Final product knitting (e.g.,socks, scarfs, caps) **Finished Product Processing (Product Printing, Product Painting, Embroidery and Embellishments, Buffing and Polishing, Moulding, etc.)** Printing and painting of products Home Furnishing: Buffing and Polishing, Moulding, etc. **Finished Product Processing (Product Dyeing, Product Laundering and Product Finishing** Dyeing of products, including wet processing, and laundering **Component / Sub-Assembly Manufacturing (including Packaging)** Label, zipper, snap, button, elastic bungee, cardboard, moulds Home Furnishing: Metal Handle Footwear: Uppers, Soles, Laces, etc. **Material Production (Raw and intermediate materials are transformed into their final state before assembly)** Fabric dye-house, fabric manufacturer, yarn dyeing, PCB manufacturer, fabric printing Hard goods: Metal pieces, wooden laminates, metal plating **Raw Material Processing (Raw Materials are processed into intermediate material products)** Yarn spinning Hard goods: Foundry, metal processing, plastic injection, wood planks **Raw Material Collection & Bulk Refining (Materials are collected/extracted/farmed and refined to bulk commodity state)** Cotton farming and ginning, processing of bottles, fabric scrap, etc. into new recycled materials, forestry, mining, crude oil refiner, MMC pul production, MMC fiber production Home Furnishing: Wooden Logs, Metal Sheet rolls 2. **Note:** If you are a facility with more than one type, you should select all that apply. For example: 1. If you are a vertically integrated facility with cut and sew operations as well as wet processing (e.g., dyeing) you will select both “Final Product Assembler” and “Finished Product Processing” 2. If you are a hardgoods facility that assembles final product and manufactures hard components on site, you will select both “Final Product Assembler” and “Component / Sub-Assembly Manufacturing (including Packaging)” **Sipproductcategories** **Product Category** *(Ref ID - sipproductcategories)* 1. Select all the applicable product categories from the list. Based on the selection(s), you will then see additional questions for more details about the specific products you manufacture. 2. Raw Material Processing **Sipmaterialbarriers** Barriers 1. This question appears if you: 1. Select this material type under sipmaterialtype in the Site section.  2. Select a facility type that is NOT “Finished Product Assembler” and/or “Finished Product Processing” 2. Barriers are any coatings and/or laminations used on textiles or footwear products. Barriers may be bicomponent (two or more materials), microporous (material with pore diameters of less than 2nm) or monolithic (single covering without seams or joints). 3. Click “Select” to choose applicable sub material types the facility uses. **Sipmaterialfoams** Foams 1. This question appears if you: 1. Select this material type under sipmaterialtype in the Site section.  2. Select a facility type that is NOT “Finished Product Assembler” and/or “Finished Product Processing” 2. Foams are a solid “open cell” or “closed cell” foam material commonly used in packaging and footwear. Includes EVA, PE, and PU foam. 3. Click “Select” to choose applicable sub material types the facility uses. **Sipmaterialinsulations** Insulation Materials 1. This question appears if you: 1. Select this material type under sipmaterialtype in the Site section.  2. Select a facility type that is NOT “Finished Product Assembler” and/or “Finished Product Processing” 2. Insulation materials are substances used to reduce or prevent the transmission of heat, sound or electricity. Insulation materials can be natural (e.g. duck/goose down, or wool) or synthetic (e.g. polyester insulation). 3. Click “Select” to choose applicable sub material types the facility uses. **Sipmaterialleather** Leather 1. This question appears if you: 1. Select this material type under sipmaterialtype in the Site section.  2. Select a facility type that is NOT “Finished Product Assembler” and/or “Finished Product Processing” 2. Click “Select” to choose applicable sub material types the facility uses. **Sipmaterialrubbers** Rubbers 1. This question appears if you: 1. Select this material type under sipmaterialtype in the Site section.  2. Select a facility type that is NOT “Finished Product Assembler” and/or “Finished Product Processing” 2. Rubbers - A tough, flexible, highly resilient, waterproof material. Natural rubber is produced using an organic compound (isoprene) usually harvested in the form of latex from rubber trees. A synthetic rubber is any artificial elastomer (polymer with elastic properties). 3. Synthetic Leathers - A synthetic (man-made and typically petroleum-based) material used as a substitute for leather. 3.
