How to Submit Data for the First Time

- Is the longest section and asks questions about your facility and operations.
- Dynamically updates the rest of the assessment based on your responses so you only have to answer questions relevant to your facility.
- Has information that rolls forward to subsequent submissions and typically only needs to be completed once.
- Enter responses for each section you selected in step 1. To upload files to questions, scroll to the right of the window to click on Choose File.
- After the FDM Assessment is 100% complete, select Submit Facility Data in the left navigation bar. The platform will only allow you to proceed once the completion rate is 100%.
